The goal of the Advancing Diabetes Self Management program at the Community Health Center was to improve the health outcomes of people with type 2 diabetes.
The diabetes self-management intervention showed patient improvements in glycemic control, blood pressure, and LDL cholesterol. The team was able to develop and adapt the program to meet the unique needs of the population to create an effective intervention.

The goal of the Al's Pals program is to teach children how to practice positive ways to express feelings, relate to others, communicate, brainstorm ideas, solve problems, and differentiate between safe and unsafe substances and situations.
Studies have shown that the program resulted in higher degrees of positive change in the intervention groups, increases in prosocial behaviors and positive coping behaviors, and decreases in antisocial and negative coping behaviors.

The goal of All Stars programs is to prevent alcohol, tobacco and drug use, postpone sexual activity, and reduce fighting and bullying among adolescents.
When teachers implemented the program, there were significant reductions in the use of alcohol, cigarettes, smokeless tobacco, and inhalants. The program also had a significant effect in changing normative beliefs, lifestyle incongruence, commitment to school, impulsive decision-making, and sensation-seeking behavior.

The goal of Assessment and Referral is to help individuals struggling with a substance use disorder evaluate their needs and strengths and begin to make healthy choices that improve the overall quality of their lives.
First Call provided over 1,700 assessments in 2013, and more than half of clients showed improvements in their Addiction Severity Index (ASI) scores or a decrease in substance use.

Home visiting can help reduce asthma triggers in the home, decreasing symptoms, missed days of work or school, and emergency room visits and hospitalizations.

The goal of ATLAS is to reduce anabolic steroid, alcohol, and other illicit drug use by adolescent male athletes.
Student participants of ATLAS had significantly lower intent to use anabolic steroids at both the end of the athletic season and at the 1-year follow-up. Students in the intervention also significantly reduced illicit drug use and were significantly less likely to report drinking and driving.
