The Ambulatory Integration of the Medical and Social (AIMS) model aims to address social and environmental factors patients face that may prevent them from following their plan of care, thus impacting their health.
The AIMS model helps create better supported, less stressed, and better informed consumers and caregivers. There is also evidence to suggest that this model reduces ED usage and 30-day readmissions in participants.

The Perfect 10 Project objective is to increase the initiation rates of breastfeeding to 84%, increase our duration rates of breastfeeding by 25% at 6 months to 35.8% (currently 28.6%), and 32.6% at one year (currently 18.1%).
There were 102 physician referrals from the Lower 8 counties for breastfeeding assistance in the first six months of 2016, 148 breastfeeding moms were helped through our after-hours hotline in the first six months, and 24 moms attended our Mommy & Me breastfeeding support group.

The AB Demonstration project successfully increased the use of health care services and reduced the reported unmet health care needs of participants in the program.

The mission of the ATHENA (Athletes Targeting Healthy Exercise & Nutrition Alternatives) program is to promote healthy sports nutrition and discourage the use of body-enhancing substances among middle and high school female athletes.
Participation in the ATHENA program results in significant reductions in the use of performance-enhancing substances, recreational drugs, diet pills, tobacco, and alcohol among female teen athletes. Healthier eating and other health behaviors, and body image perceptions were also improved.

The Carrera Adolescent Pregnancy Prevention Program's goal is to provide comprehensive youth development services and reduce teen pregnancy among economically disadvantaged teenagers.
Pregnancy prevention programs can work successfully among females when started early in adolescence and when male counterparts are also educated appropriately on condom-use and delayed sexual actively onset.

To modify adolescents' risk and protective behaviors by improving their caregivers' parenting skills based on sufficient evidence of effectiveness in reducing adolescent risk behaviors.
Although the estimated effects varied substantially and were not statistically significant, risk behaviors decreased and youth participants reported increased refusal skills and self efficacy for avoiding risky behaviors in the future.
