Priority Issue: Prevention and Management of Serious Illness
How do we build a person-centered model of healthcare that is efficient, effective, equitable and creates meaningful access to care for all and demonstrably improves health status?
Impact DuPage is partnering with two coalitions to address the Prevention and Management of Serious Illness: DuPage Health Coalition and DuPage Health Equity and Access Response Team (HEART). These organizations will work to address the needs for meaningful access and improved health outcomes in DuPage County.

DuPage Health Coalition
Mission: To develop and sustain in DuPage County a system for effectively and efficiently managing the health of low-income populations across the continuum of care.
Through a joint strategic planning process, the DHC and the DuPage Federation on Human Services Reform developed the DuPage Safety Net Plan for Health and Human Services 2016-2018.
